University ID (UVID)

Use this form to request your name changed to a preferred name. For staff and faculty users only who do not have LOCUS access.

This is a request form for users to be added to Reporting Services Active Directory Groups. All reporting services AD groups have _RS_ or some variant in the name.

Use this form for one these reasons: spelling correction, update UVID(username) after a legal name change, or to shorten your existing UVID.
